SUBMIT ONLY ONE APPLICATION PER CHILD*
Post Office box addresses will not be accepted for the student's address. You MUST have a resident address, with house number, street, city, and zip code for the application to be accepted. If you are completing a new application or if there is no preprinted information in Section I, enter all information requested. Since applications will be electronically read, please print clearly in CAPITAL LETTERS, using black or blue ink.

Verify/Enter student information
For personalized applications, please verify that the information is accurate.  If the information is accurate, you may continue. If it is not, you should contact the child's LAUSD school to update the student information.  Once you have done so, you may fill out the application and submit it.
If your child is presently attending a private or charter school, please contact the Office of Student Integration Services at (213) 241-4177 to make any changes.
• Post Office box addresses will not be accepted for the student’s address. You MUST have a resident address with house number, street, city, and zip code for the application to be accepted.
• Any information changes must be corrected at the current LAUSD school of attendance. The following can be used to verify current address: Original utility bill such as DWP, Edison, Gas Co., Tax bill or rent/lease agreements. Telephone bills will not be accepted. 
• Private/charter school applicants with any information changes should call (213) 241-4177.
For blank applications make sure that you fill out all fields legibly and completely using black or blue ink. This information will be entered electronically and used to identify the student.
 
Ethnicity information
For participation in the Magnet or PWT Programs, which are Court-Ordered, you MUST mark ONE (1-7) federally identified race/ethnicity category for the purpose of selection. You may also fill in the “Multi-racial/Multi-ethnic” bubble to indicate the applicant is identified as more than one race/ethnicity. Your application WILL NOT be processed unless a single ethnicity (1-7) is indicated.
AMERICAN INDIAN A person having origins in the original peoples of North America, who maintains cultural identification through tribal/community affiliation.
ASIAN A person having origins in the original peoples of the Far East, Southeast Asia, or the Indian Subcontinent, e.g., China, India, Japan and Korea.
BLACK/AFRICAN AMERICAN, NOT OF HISPANIC ORIGIN A non-Hispanic person having origins in any of the Black racial groups of Africa.
HISPANIC A person of Mexican, Puerto Rican, Cuban, Central or South American or other Spanish culture or origin, regardless of race.
WHITE A non-Hispanic person having origins in any of the original peoples of Europe, North Africa or the Middle East, e.g., England, Egypt, Russia or Iran.
FILIPINO A person having origins in any of the original peoples of the Philippines.
PACIFIC ISLANDER A person having origins in any of the original peoples of Polynesia, Micronesia or Melanesia.
 
Choose ONLY ONE program
The choices are Magnet, PWT, or PSC.  You MUST choose ONLY one program.  Choosing more than one program will cause the applicant to receive a correction letter.
Select Magnet Schools/centers.
Check Permits With Transportation (PWT) program eligibility and school assignment.
Check Public School Choice (PSC) eligibility.
 
Sibling information
If the applicant has a brother or sister currently enrolled in the same Magnet school, PWT or Public School Choice program for which he/she is applying and who will be continuing next year, please fill in the sibling’s last name, first name, middle initial and birth date. Siblings must reside at the same address.
 
Information Sharing
Depending on seat availability at the particular Magnet or PWT program you have selected, your child may be placed on a waiting list for the 2008-2009 school year. Other Magnet programs of possible interest to you may have space available for the 2008-2009 school year. With your consent (by marking the “Yes” box), your contact information will be provided to other Magnet program staff who may contact you about alternative Magnet placements for your child. This information will be kept confidential and will not be used for any other purpose. You are under no obligation to accept any alternative Magnet placement offered to you. Your child’s name will remain on the current waiting list unless you select an alternative Magnet placement.
 
Parent Signature
Your signature verifies that all information submitted on the application is correct. If the applicant is selected, your signature authorizes the transfer of student records to the Magnet/PWT/PSC school. This application must be signed in black or blue ink.

Falsification of information will result in the applicant’s removal from the program. It is the responsibility of the parent or legal guardian to complete the application.
